Learning and or Behavioral Issues

As parents you are the best people to determine whether you feel your child might have a learning disability or a behavioral issue. You may feel overwhelmed by the information, the numerous tests, and the lack of clarity and direction available to you to get the most effective help for your child. Early intervention and support through home, school, our offices and in some cases, specialists, can really make a difference in giving your child the best chance for success.

An appointment with us to begin this process is the first step to take. In order for your child’s physician to better understand the issues you are concerned with, we ask that you assist us in obtaining the following information. We further believe that it is in the best interest of the child for both parents to be present at the initial appointment.

1. A separate Parent’s Questionnaire completed by each parent.
2. A completed teachers questionnaire.
3. A self questionnaire depending on age of child.
4. Copies of any reports done by other specialists.
5. Classroom Observation Reports.
6. Copies of past and current report cards.
7. Any Teacher comments.
8. Any additional reports or educational testing from school.
9. Additional information relating to concerns from other sources.

We ask that parents gather this information and return it to us as soon as possible. After your child’s doctor has had time to review the reports, we will be able to schedule an appropriate appointment time to address your concerns.
